s current condition and accepts said Report as part of the Agreement with Management. Resident also
accepts the responsibility to notify Management in writing by certified mail of any findings not herein
listed within seven (7) days after Resident receives possession. Failure to do so shall be Resident's
acknowledgment that Property is in the above described condition in every particular and is in perfect

I/We, as Resident, agree to pay any and all above charges for the above listed just causes in
addition to any other items furnished to Resident. Labor charges include installation, cleaning,
patching, sanding, etc., including the cost of material and cleansers. If Resident does not clean or
repair the damaged or soiled items prior to moving out, charges will be billed to Resident based on
the above schedule. Items not on the list will be charged on a “cost plus labor” basis. Fees that are
not collected from Resident will be forwarded to an attorney and/or collection agency for recovery.
By initialing this Move Out Fee schedule and entering into this Lease, Resident agrees to the Fees
as stated above and accept said Fees as part of the Lease of this Premise with Management.

Lead Warning Statement: Housing built before 1978 may contain lead-based paint. Lead from
paint, paint chips, and dust can pose health hazards if not taken care of properly. Lead exposure
is especially harmful to young children and pregnant women. Before renting pre-1978 housing,
landlords must disclose the presence of known lead based paint and lead-based paint hazards in
the dwelling. Tenants must also receive a Federally approved pamphlet on lead poisoning
prevention.

MOVE OUT PROCEDURES
If you move out of the home please use this checklist to insure a smooth move out and correct and
fair charges.
1. ___ If not already done so on your Notice To Vacate form, schedule the move-out inspection
with Management 5 days in advance of the date you want.
2. ___ All floors swept, vacuumed, washed, and waxed.
3. ___ All walls & ceilings dusted down & all dirt, smudges, grease, cigarette stain washed off
4. ___ Wash down all baseboards, woodwork, and windowsills.
5. ___ Clean and wash all light fixtures.
6. ___ Bathroom; thoroughly clean all fixtures-toilet(s), bathtub(s), showers, sinks, cabinets.
7. ___ Kitchen; clean, wash inside & outside, removing all dirt & grease including kitchen sink &
fixtures
8. ___ Keep all utilities on until day after your move-out inspection is complete.
9. ___ Clean behind & between stove and refrigerator area.
10. ___ Clean storage area.
11. ___ Sweep & wash hallway floors and dust hallway walls.
12. ___ Vacuum & clean carpets.
13. ___ Clean closets, shelves, & rods wiped down.
14. ___ Clean all windows, storms, screens, and return them to their proper place.
15. ___ Remove all items from attic, crawlspace, basement, yard, shed, garage, etc.
16. ___ Lawn, driveway, garage, outbuildings, to be free of all trash, rubbish, and personal
property.
17. ___ Lawn trimmed and cut properly, including removal of leaves.
18. ___ All trash and garbage to be removed from property. DO NOT PILE up garbage at the
curb.
19. ___ Remove all garbage from property before your inspection.
20. ___ Do not remove phone jacks, picture hooks, or curtain rods and brackets.
21. ___ All keys are to be returned to staff or as instructed.
22. ___ LIGHT BULBS - ALL light fixtures are to have working light bulbs in ALL bulb sockets.
23. ___ Smoke alarm(s) will be in working order with good battery.
24. ___ General; The entire dwelling, appliances, closets, cupboards are clean and free from
insects, the refrigerator is defrosted, all debris and rubbish have been removed from the property.
25. ___ A forwarding address has been left with the Landlord. Tenant must notify Landlord in
writing of a forwarding address where Tenant will receive mail after Tenant’s move.
Any expenses associated with returning the condition of the Property to its move-in condition, as
stating and agreed to in the Inspection of Property Condition Report can be deducted from your
security deposit. What your security deposit does not cover YOU WILL BE CHARGED AND
REQUIRED TO PAY. If charges are not paid in 45 days after move out court collections will begin.
Your security deposit and/or deposit disposition breakdown will be mailed to the forwarding address
you provide no later than 30 days after your Move-out inspection has been completed.
Page 12 of this Lease is your Repair Fee Schedule listing charges you will incur you do not return
your home in the same condition as when you moved in as listed in the Inspection of Property
Condition Report.
